Report a Positive COVID Test

At Foothill, we are required by Cal/OSHA to report all cases of a positive COVID test, including students and employees, through Feb. 3, 2025.

Students

Please use the COVID reporting form as soon as possible. Faculty/staff should not use the above form for students as they need to self-report. 

Staff & Faculty

Employees who test positive for COVID or have been exposed on-campus should follow district guidelines and protocols.

In particular, employees need to report both their own COVID positive test or an exposure to COVID while on campus using this form. Note: This is the correct form; De Anza just hosts the form on their site.
