PLEASE READ the program application information and instructions carefully.

3. Schedule an appointment with a Foothill College counselor to determine which General Education (GE) pattern applies to you.
   ---> You may schedule an appointment here: Counseling Services
   ---> When scheduling your appointment, inform the counselor that you are a working dental hygienist interested in the AS to BSDH Completion Program and would like to determine which GE pattern best applies to you.
   ---> We highly recommend that you have your unofficial transcripts available during your appointment.
4. GE Worksheet 
   ---> Beginning in Summer 2025, the General Education (GE) requirements have been updated. Submission of a GE worksheet is no longer required with the application; however, applicants are highly encouraged to meet with a Foothill College counselor to confirm the GE pattern that applies to their academic plan.
5. GE Certification (optional) - if you have completed the GE requirements, you may send this letter to your previous school counselor to request a GE certification to submit with your application.

7. Read the online application checklist carefully. 
8. Make sure your application is complete and that you've attached ALL the required documents before submitting it. If your application is incomplete upon receipt, it will result in delayed processing or may disqualify your application.
